        West Sussex County Council (WSCC) is seeking a Service Provider to deliver an integrated 0-19 Healthy Child Programme (HCP) for the county of West Sussex. The successful Service Provider will be able to demonstrate a track record of delivering HCP services, and act as a collaborative partner with the Council as it looks to provide a fully Integrated Prevention and Earliest Help service (IPEH).

        The Healthy Child Programme (HCP) 0-19/25 is a research-evidenced population-based public health programme starting during pregnancy and continuing throughout the early years and school life of children and young people. It is a progressive universal model, delivering an offer to all families with children and a targeted and enhanced service to those with an identified need. Young people with special educational needs and disabilities continue to be eligible for the HCP up to the age of 25. There is a strong emphasis on building resilience, early identification and prevention of escalating health and social care need in children and their families.

        West Sussex County Council (WSCC) has had responsibility for commissioning of the HCP (5-19) since 2013. Commissioning responsibility for the HCP (0-5), including Family Nurse Partnership (FNP), transferred to the Council on 1.10.2015 from NHS England. Current contractual arrangements expire on 31.3.2017, providing an opportunity to review and redesign services. A comprehensive stakeholder engagement process has taken place to inform the redesign, from which key themes have emerged that have informed the development of a vision and future service model.
